Purpose
Objective severity scoring of atopic dermatitis combining extent (A), clinical sign intensity (B), and patient-reported symptoms (C).
Population
All ages. Modified rule of nines applies to infants under 2 years.
Formula
SCORAD = A/5 + 7×B/2 + C   (max. 103)
Severity thresholds
Mild < 25  ·  Moderate 25–50  ·  Severe > 50
📐 Part A — Extent

Select affected body regions. Dry skin without active inflammation does not count.

Body surface area affected 0%
🔬 Part B — Intensity

Score each sign on a representative affected area. 0 = Absent · 1 = Mild · 2 = Moderate · 3 = Severe

Intensity total (B) 0 / 18
💬 Part C — Symptoms

Patient-reported symptoms over the last 3 days/nights. 0 = none · 10 = worst imaginable.

0
None Worst imaginable
0
None Worst imaginable
Symptoms total (C) 0 / 20
